Non-Insurance Covered Surgery Quote Policy

Quotes for surgery performed by Mark D. Wigod, M.D., P.A. at Meadow Lake Surgery Center (an accredited ambulatory surgery center) include:

• Surgeon’s fee
• Operating room fee
• Materials
• Anesthesia fee

This quote is valid for 6 months. It does not include pathology or unanticipated costs — these remain the patient’s responsibility. Pathology specimen submission may or may not be required and may or may not be covered by health insurance.

Quotes for surgery performed at an outside facility cover the surgeon’s fee only. This quote is also valid for 6 months. All other costs — including anesthesia, OR, hospital, special materials, pathology, and unanticipated costs — are not included and are the patient’s responsibility to arrange and pay. Additional hospital and anesthesia fees may apply if surgery takes longer than planned.

Refund Policy

Exception: Specially ordered materials (e.g., injectable products, implantable devices) that cannot be returned are non-refundable regardless of cancellation timing.

Note: Services paid by credit card, debit card, or financing are not eligible for post-care payment challenges (chargebacks).

Rescheduling Policy

The practice reserves the right to refuse a rescheduling request.

Revision Policy

• If a revision is needed to achieve a result considered acceptable by both surgeon and patient, it is typically provided without a surgeon’s fee.
• If the revision requires significant resources (anesthesia, OR, hospital, special materials, pathology, or unexpected costs), the patient is responsible for those costs.
• Patients are responsible for all revision fees if they:
  • Chose a treatment option not recommended by the surgeon
  • Did not follow peri-operative instructions
  • Experienced a problem outside the surgeon’s control
• For additional surgeries requested to improve on an already-acceptable outcome, fees may be full or reduced at the surgeon’s discretion.
• Any fee waiver or reduction is a courtesy and does not indicate an admission of negligence or substandard care.

Emergency/After-Hours Care: If urgent care is needed outside regular business hours, or a complication cannot be managed as an outpatient, care will be transferred to Meridian St. Luke’s Medical Center. Patients are responsible for any resulting hospital charges. Health insurance does not cover these costs for cosmetic surgery patients.

Insurance-Covered Surgery

Mark D. Wigod, M.D., P.A. and Meadow Lake Surgery Center do not bill insurance for any services. If a procedure is medically indicated, patients may request insurance coverage themselves, and the practice will assist with documentation and billing codes as needed.
