The patient is a woman in her 60’s who had a surgical large weight loss. She was dissatisfied with her abdomen and desired abdominoplasty. As she had vertical as well as horizontal redundant tissue, she accepted an added vertical scar to the traditional horizontal scar near the pubic hair line. The inverted T closure is difficult to construct while maintaining good blood flow and a level scar, but the result can be rewarding in appropriately selected patients.
